Consent to Treat

1. Purpose of This Consent
This Consent to Treat describes the terms under which you agree to receive IV therapy and related wellness services booked through SurgIV. By checking the consent box during checkout, or by receiving treatment from a SurgIV-affiliated clinician, you acknowledge that you have read and agree to this Consent, in addition to our Terms of Service.
2. Nature of Services
SurgIV coordinates in-home IV therapy delivered by independent, licensed registered nurses under physician-written protocols. There are no guaranteed results from any treatment. Your clinician will explain the intended purpose and expected effects of your selected treatment before administering it.
3. Health Screening & Good Faith Exam
Before any treatment is administered, a licensed clinician will conduct a brief health screening (Good Faith Exam) to confirm you are a safe candidate for IV therapy. You agree to answer all screening questions truthfully and to disclose your complete relevant medical history, current medications, supplements, and allergies. Failure to disclose accurate information may affect the safety and outcome of your treatment, and SurgIV and its affiliated clinicians are not responsible for outcomes resulting from incomplete or inaccurate disclosures.
4. Potential Risks
As with any IV therapy, there are potential risks, including but not limited to minor bruising or discomfort at the injection site, vein irritation, or allergic reaction. Your clinician will discuss relevant risks and benefits with you so you can make an informed decision about whether to proceed.
5. Pregnancy & Breastfeeding Notice
Some IV formulations may not be appropriate during pregnancy or while breastfeeding. If you are pregnant, believe you may be pregnant, are planning to become pregnant, or are breastfeeding, you agree to notify your clinician before treatment begins.
6. Right to Withdraw Consent
You may withdraw your consent to treatment at any time, including at the start of your appointment, before treatment begins, by informing your clinician. Standard cancellation terms described in our Cancellation Policy will still apply.
7. Financial Responsibility
By booking a service, you acknowledge financial responsibility for the full cost of the treatment and any add-ons, booster shots, or fees (including the flat $100 travel fee) selected at checkout, on a cash-pay basis outside of any insurance program.
8. Acknowledgment
By proceeding with a SurgIV booking, you acknowledge that you have read this Consent to Treat, understand its terms, and voluntarily agree to receive treatment under the conditions described above.
