1. Agreement to Terms

These Terms of Service ("Terms") govern your access to and use of the SurgIV website, booking flow, and related services (collectively, the "Service"), operated by SurgIV ("SurgIV," "we," "us," or "our"). By browsing this site, submitting a booking request, or otherwise using the Service, you agree to be bound by these Terms. If you do not agree, please do not use the Service.

2. Who We Are

SurgIV is a concierge booking platform that connects you with independent, licensed registered nurses and a supervising medical team who deliver in-home IV therapy and related wellness services. SurgIV itself is not a hospital, clinic, or medical practice — we operate the booking platform and coordinate scheduling, while the actual clinical services are provided by licensed clinicians affiliated with SurgIV.

3. Medical Disclaimer

The Service is not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Content on this site — including treatment descriptions, ingredient lists, and general wellness information — is provided for informational purposes only and does not constitute medical advice. A Good Faith Exam (GFE) and medical screening by a licensed clinician is required before any IV treatment is administered, and that clinician has sole discretion over whether a treatment is appropriate for you.

4. Eligibility

You must be at least 18 years old to book a service through SurgIV. By submitting a booking, you represent that you meet this requirement and that the information you provide is accurate and complete.

5. No Insurance Billing

SurgIV does not bill insurance, Medicare, or Medicaid for any services. All treatments are offered on a cash-pay basis. You are solely responsible for the full cost of any service you book, including the flat $100 travel fee applied to every in-home visit.

6. Booking & Guest Checkout

Our booking flow currently supports guest checkout — you do not need to create a persistent account to request an appointment. Submitting the booking form constitutes a request for service, not a confirmed, guaranteed appointment time; a member of our team will follow up to confirm scheduling, complete any required health screening, and finalize payment before your visit.

7. Memberships

SurgIV offers optional recurring membership programs (such as The Surge Protocol) that provide member pricing on future bookings. Membership terms, billing cadence, and cancellation windows are disclosed at the time of signup and are also governed by these Terms.

8. Cancellations & Refunds

Cancellation, rescheduling, and refund rules — including applicable fees for late cancellations or missed appointments — are described in our Cancellation Policy, which is incorporated into these Terms by reference.

9. Prohibited Conduct

• Providing false or misleading health information to our clinical team.
• Booking services on behalf of another adult without their knowledge and consent.
• Using the Service for any unlawful purpose or in a way that could harm SurgIV, our clinicians, or other users.

10. Intellectual Property

All content on this site — including the SurgIV name, logo, treatment names, and site design — is owned by SurgIV or its licensors and may not be copied or reused without our written permission.

11. Limitation of Liability

To the fullest extent permitted by law, SurgIV is not liable for any indirect, incidental, or consequential damages arising from your use of the Service. Because clinical care is delivered by independent licensed clinicians, SurgIV is not liable for the outcome of any medical treatment; any claims relating to the treatment itself should be directed to the treating clinician.

12. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any disputes arising under these Terms will be resolved in the state or federal courts located in Palm Beach County, Florida.

13. Changes to These Terms

We may update these Terms from time to time. Changes take effect once posted to this page, and your continued use of the Service after changes are posted constitutes acceptance of the updated Terms.
